Must bring for each person in their household each time you come. 

  1. Proof of address 

    • A utility bill  

    • Lease Agreement or mortgage statement.

    • Insurance Card.

    • Voter Registration Card.

    • College Enrollment Papers.

  2.  A government-issued ID 

    • birth certificate

    • social security card 

    • passport

    • Tenncare, 

    • student ID
